Our Y-Prep Program serves over 400 children and their families by providing clinical services and Targeted Case Management to children ages 5 to 17 with a mental health diagnosis in a nurturing, supportive and therapeutic environment. We provide transportation from school and to home for all children to ensure that lack of transportation does not hinder them from receiving the mental health services they need to enhance positive development. Our group facilitators and therapists are highly skilled in the fields of Education, Mental Health Counseling, Social Work, Psychology and Psycho-social Rehabilitation through group practices. We possess a vast experience working with children with mental health diagnoses. Group facilitators must possess a minimum of a bachelor’s degree in a social service related field and therapists must have a master’s degree in Psychology, Social Work and/or Mental Health Counseling. Our Y-Prep Program has a Licensed Clinical Social Worker on staff and all master’s level therapists are Registered Clinical Social Worker Interns working towards licensure. Children receive homework assistance and meals within the program. Family counseling services are also provided because at Community Connections for Life, CMHC we understand the importance of supporting and strengthening the system to in turn, address and support the needs of the child.
