Located in the beautiful and diverse community of South Florida, Community Connections for Life, CMHC is a Non-for-profit 501(C) 3 Community Mental Health Center, designed to address the needs of those living with mental illness in South Florida. We believe in aiding those we serve with love, compassion, and respect.
Community Connections for Life, CMHC emerged from the dire need for quality mental health services in our local communities. Our organization provides a vast array of services to individuals, ages 5 and up, living with severe and persistent mental illness. Services include Behavior Modification Groups for children and adults, Psychosocial Rehabilitation, Targeted Case Management, Counseling and Psychiatric services. These services are provided at our CMHC, public and private school settings, and in the community, with the goal of enhancing the level of functioning of our clients by taking a holistic approach that will not only treat their symptoms but treat the whole person in a therapeutic, warm, exciting and safe environment.
Behavior Modification is a reparative strategy that replaces or "modifies" maladaptive behaviors with more desired, prosocial behaviors through the implementation of evidence based interventions. Our goal is to assist and support individuals with making fundamental changes as they develop emotionally, cognitively, biologically, and socially. Behavior modification intervention groups include but are not limited to anger management, leadership skill development, self-esteem building, positive thinking and goal setting, effective decision making skills, among others. Positive and negative reinforcement strategies are used to increase positive behaviors and decrease negative ones. In addition, Cognitive Behavioral Therapy (CBT) is often used to change unhelpful behaviors and thoughts that may be negatively affecting mental and emotional wellbeing.
Our PSR program focuses on improving the quality of life for individuals living with mental health issues by restoring their ability to function in their homes, school and within the community. Our PSR program may last anywhere between 6 to 12 months depending on need and level of functioning. PSR is available 5 days a week and is tailored to restoring the social skills and abilities of our clients using psycho-education, peer support, and mutual aide amongst participants. PSR groups were developed to assist our clients in building the skills needed to achieve optimal levels of functioning within society while reducing symptoms of mental health and placing emphasis on their strengths and capabilities.
At Community Connections for Life, we believe that ALL people are valuable and unique and should be treated with dignity and respect. While recognizing the potential for change, our professional staff utilizes their knowledge and skills to assess the client’s emotional, physical, spiritual, and life experiences to determine the best practices. The growth and wellness of the individual is promoted through a course of therapeutic treatment and is executed in a timely manner, in line with the client’s goals. Clients at Community Connections for Life receive Targeted Case Management Services and are also assigned a Therapist to ensure that care and support are provided at different levels and to address a variety of needs. Clients participate in the development of their care plan and treatment goals and are given options regarding available services, service providers, and potential service outcomes to ensure that treatment is client centered and tailored to the client and not the diagnosis.
We pride ourselves in going the extra mile to ensure that our clients receive the services that they need by providing transportation, meals and basic necessities as needed. We are conveniently located to serve Miami-Dade, Broward, and Palm Beach Counties.
We service the community and Medicaid recipients who have a serious emotional disturbance and/or suffer from chronic mental illness such as Major Depression, Anxiety, Personality Disorder, Bipolar Disorder Schizophrenia, Post-Traumatic Stress Disorder, Attention Deficit Hyperactivity Disorder, Obsessive Compulsive Disorder, Oppositional Defiant Disorder, and Conduct Disorder among others.
